I hope this message finds you well. At York Medical Group, we are constantly looking for ways to improve our services and provide better care to our patients. As part of this commitment, we are planning to introduce a new digital webform that will help us manage long term conditions more effectively.

We are excited about this new project and I would like to invite you to get involved in the development process. Your input and feedback are critical to the success of this project, and we believe that involving patients in the design and development of this new tool is the key to ensuring that it meets your needs and expectations.

To that end, I would like to invite you to test a beta version of the webform to provide us with your feedback. Your input will help us fine-tune the webform and ensure that it is easy to use, reliable and effective. We value your time and commitment, and I want to make the process as easy and straightforward as possible for you. 3You will be asked questions about your (pretend) long-term conditions. For demonstration purposes you will be asked questions about multiple conditions. Please give ‘dummy’ answers to test the form. You do not need to provide real information. The form will ask questions about conditions that may not actually have. Please note that the final version of the form will only ask questions related to you.  
4The last ‘thank you’ page will present you with an option to leave feedback. Please consider sharing your experience. All feedback is anonymous.
